Among couples who cannot conceive naturally within one year of marriage, approximately 15% face fertility issues, with 40% of these issues related to males. The most pressing concern for these male infertility patients is how to undergo effective treatment.

The most crucial diagnostic tool is the semen analysis. Through this test, the patient's fertility can be evaluated, and diagnoses such as oligozoospermia, asthenozoospermia, teratozoospermia, and azoospermia can be made. However, relying solely on semen analysis is insufficient because it only assesses fertility levels without pinpointing the specific causes of reduced fertility or providing enough information for treatment planning. Therefore, a series of specific tests are needed to identify the reasons for abnormal sperm parameters.

Currently, clinically relevant tests for male infertility include:

1. Sperm membrane surface antibodies: Detects whether male infertility patients have immune factors.

2. Seminal fructose and glycosidase tests: Assess the function of accessory glands and the presence of obstructive lesions in the ejaculatory duct.

3. Hormone tests: Determines whether patients have endocrine disorders.

4. Seminal elastase and semen leukocyte staining elastase: Combined with semen leukocyte staining, it can determine the presence of reproductive tract inflammation.

5. Seminal acid phosphatase test: Important for patients with poor semen liquefaction.

6. Seminal lactate dehydrogenase test: Assists in assessing sperm quality and predicting fertilization capacity.

7. Acrosomal enzyme test: Mainly assesses the fertilization capacity of sperm.

8. Chromosome and spermatogenesis factor tests: Aim to evaluate whether male infertility patients have congenital factors.

9. Seminal zinc detection: Zinc serves as a cofactor for over a hundred enzymes in the body. Seminal zinc testing provides guidance for the treatment of male infertility.

Specific tests should be arranged individually by clinical doctors based on the patient's actual situation. Through comprehensive testing, some patients may identify the reasons for abnormal semen analysis results and receive corresponding treatment, which is more likely to be effective. If appropriate treatment still does not result in pregnancy, assisted reproductive technologies such as artificial insemination or IVF need to be considered.
